VBbrides LLC (“VBbrides”) is excited that you are interested in enrolling in our program as you strive towards your special day!  Please review the following terms of your enrollment agreement.   

  1. Non-Refundable Payment:  This program is non-refundable.  In the event that you agree to the terms herein and make payment for a VBbrides package, please note that all payments are non-refundable.  It is, of course, our hope that you will complete the entire program.   

  2. Assumption of Risk, Waiver and Release of Liability:  You will be expected to partake in physical workouts as part of the VBbrides program.  Accordingly, you understand that there are certain inherent risks associated with these workouts.  You acknowledge and agree that you assume the risks associated with any and all activities and/or exercises in which you participate as part of the VBbrides program.  You hereby assume full responsibility for any and all personal injuries to yourself and others while completing any workout associated with the VBbrides program.  You hereby agree that VBbrides is not responsible or liable for any injury, loss, or damage arising out of the workouts you complete as part of its program.   

  3. HIPAA Notice of Privacy Practices: This HIPAA Notice describes how protected health information about you may be used and disclosed by VBbrides.  Depending on the VBbrides package you select, you may be working closely with a Registered Dietician (“RD”).  During this process, it is expected that you will provide the RD with protected health information.  Please note that VBbrides respects your privacy and understands that your health is personal, and is committed to protecting health information about you.  This HIPAA Notice applies to all records of your care generated by an RD whether made by our personnel or your personal doctor.  Please note that VBbrides will only disclose protected health information about you when: (a) required to do so by federal, state, or local law; (b) when necessary to prevent a serious threat to your health and safety or the health and safety of the public or another person; and (c) in response to a court order, administrative order, or subpoena.  Please feel free to contact us with any questions regarding this HIPAA Notice.
